PLEASE ENSURE YOU FILL ALL THE MANDATORY FIELDS ON THE FORM (THOSE MARKED WITH AN ASTERISK) OR IT WILL NOT SUBMIT AND WILL NOT BE RECEIVED BY THE MATERNITY BOOKINGS TEAM. IF YOU SUBMISSION HAS BEEN SUCCESSFUL YOU WILL RECEIVE A RESPONSE PAGE BACK IMMEDIATELY.

If you require any support with our online form please call our designated phone number 01823 343321. This is an answerphone and we may take up to three working days to get back to you. Please leave your contact details and a member of our team will get back to you.

If you are currently taking any regular medications we recommend making an appointment with your GP as soon as possible following a positive pregnancy test to review these.

We also recommend taking a daily dose of folic acid (400 micrograms ideally) at least three months before pregnancy and a dose of vitamin D – 10 micrograms. If you wish to take these as part of a pregnancy multivitamin tablet, make sure they don’t contain vitamin A (or retinol). These can be bought at any supermarket or pharmacy. Some women may require a higher dose of folic acid (5mg) – this can only be prescribed by a GP. For more information regarding this please view both links below:
