Patient Experience Survey

Tell us about your experience of accessing your GP practice

Our practice is committed to ensuring you can access the advice and support you need from us and we want to understand your experience of doing so.

So that we can be sure we are doing all we can to provide the support you need, we are taking part in testing a national survey to help us better understand your experience. This means following your appointment you may receive a text message on your mobile phone inviting you to respond to a short, online survey.

Your answers will remain entirely anonymous but provide us with the information we need to understand what has worked well and how we might improve your access to health and care support.

We would like to encourage all of you to take part in this survey if you are given the opportunity. As this is a trial of the process, your input will also help us to develop an approach which can be extended to all GP practices in England. This will include other ways of collecting feedback from patients, including those who do not have online access or may need alternative formats to participate in the survey.

If you have recently had an appointment, or you’ve spoken to us on the phone or online, we will use some of the information we hold about you (including your mobile number and things like your gender and date of birth) to enable you to receive the survey. So that your data is safe, we have ensured all required information governance safeguards are in place and if you wish, you can opt out of receiving the survey at any time.
