How to register with us

We are currently accepting new patients. If you wish to register at Warwick House Medical Practice, you can either collect a new patient registration pack from reception, complete it and return to the practice or download the following forms to complete at home.

It is helpful if you are able to provide proof of address at the time of registration so that we can ensure that you are living within our practice boundary. However you will not be refused registration if you do not have this. You do not need to have proof of immigration status, ID or an NHS number to register with a GP.

If, for any reason, your registration is not accepted or if we need further information to process it, we will contact you.

  1. Practice Registration form
  2. Health information form.  There are two different forms; one for adults (any over the age of 18 years) and and one for children (under 18 years). Please note you will need to complete a form for each person you are registering. 

If you are on multiple medications, it would be useful to book an appointment with our practice pharmacist for a medication review soon after you join us to ensure we have the correct information when you need to request your next prescription.

Please be aware that we do not register temporary patients for the sole purpose of obtaining repeat medication. Please contact your registered practice who will send an electronic prescription to a local pharmacy of your choice.
