If you are already set up for Patient Access or the NHS app just you can contact us to request your Linkage Key and codes by submitting an admin query through our online consultation service

If you are not set up for the GP online services you can either register direct in the NHS App or complete and return the online access request form together with a photo of your ID to [email protected]
